GENERAL OFFICE POLICIES


OFFICE HOURS:
The office is open Monday through Friday, 9:00AM to 4:00PM


APPOINTMENTS AND CANCELLATIONS:
We ask that all routine appointments be made in advance. If you find you are unable  to keep  your   appointment, please notify our office 24
hours in advance. This courtesy on your part makes it possible to give an appointment  to another patient who desires to see the doctor.

FORMS:
For your convenience we have our standard office forms available online. Patients may download  and fill  these forms out prior to their
office visit. Please note depending on their nature of consultation, it may be necessary to complete additional forms upon their arrival in
the office.

PRESCRIPTIONS:
For refills, we ask that you provide at least 72 hours notice. Please call the pharmacy and request them to call our office for authorization on
the refill. This will facilitate your request in a more timely manner. The Clinical  staff   will submit your request to your physician. Once the refill
has been authorized, it will be called to your  pharmacy.  If  there is a problem with the request, you will be notified by the Clinical staff.

TEST RESULTS:
Please call our office 3-4 days after the date of the test.

PATIENT REQUEST FOR MEDICAL RECORDS:
We will need a signed authorization from the patient for release of medical records. We will accept faxed authorizations.
